The adult intake paperwork may better suit the needs of older children and teens.

Cancellation Policy

When you reserve an appointment at Origin Chiropractic, you are holding a space no longer available to other clients. In order to best serve our community, we ask that you kindly contact the office as soon as possible with any scheduling changes.

For regular visits, please cancel at least 8 hours in advance. No-show appointments and late cancels (less than 4 hours) will be subject to fees. For complete details, see our initial visit paperwork (in office) and our confirmation emails. Thank you!
